1. Website privacy vs. your medical information (HIPAA)
This policy covers information collected through our website and general inquiries. It does not cover your medical records.
When you become a patient, your protected health information (for example, visit notes, test results, prescriptions and messages in our patient portal, and information shared during in-person or telemedicine visits) is protected by the federal HIPAA Privacy Rule and other applicable laws. It is described in our separate Notice of Privacy Practices. That notice explains how we may use and disclose your medical information and your rights, such as the right to see and get a copy of your records.
A copy of our Notice of Privacy Practices is available on request at our office or by contacting us (see Contact us). We also provide it to new patients.
2. Information we collect
Information you give us
- Contact form: your name, email address, phone number and the message you write.
- Phone, text and email: if you call, leave a voicemail, text us at 984-212-3686 or email us, we receive your phone number or email address and whatever you choose to tell us.
- Email list: as stated on our contact form, giving us your email address on the form adds you to our email list for occasional practice news and educational information. You can unsubscribe at any time.
Information collected automatically
Like most websites, our website and its service providers automatically collect some technical information when you visit, such as your IP address, browser and device type, the pages you view, the date and time of your visit, and the website that referred you. See Cookies, analytics and similar technologies.
Patient portal and telemedicine
Our patient portal and telemedicine platform are separate secure systems operated by third-party health technology vendors. They are not part of this website. Information you enter there is medical information covered by our Notice of Privacy Practices, not by this policy.
3. How we use information
- To respond to your questions, schedule appointments and send appointment-related messages.
- To send you emails you have agreed to receive. You can unsubscribe at any time.
- To operate, maintain, secure and improve the website, including protecting our contact form from spam and abuse.
- To understand, in general and in aggregate, how visitors use the website, for example which pages are visited most.
- To comply with the law and to protect the rights, safety and property of our patients, our practice and others.
4. Cookies, analytics and similar technologies
Cookies are small files stored on your device. Similar technologies, such as scripts and pixels, do comparable things. Our website uses:
- Google tag (Google Analytics). We use a Google tag, added through Google’s Site Kit plugin for WordPress, to measure website traffic and understand how the website is used. Google may set cookies and receive technical information about your visit, such as your IP address, device and browser information and the pages you view. Google’s use of this information is governed by Google’s Privacy Policy. You can learn more at How Google uses information from sites that use its services, and you can opt out of Google Analytics with the Google Analytics Opt-out Browser Add-on. This tag is part of our public website only. It is not part of our patient portal or telemedicine platform.
- Spam protection. Our contact form uses Cloudflare Turnstile to tell real visitors from automated bots. Turnstile processes technical information about your browser and device for this purpose, as described in Cloudflare’s Privacy Policy.
- Essential functions. Our website software (WordPress) and its plugins may use cookies or similar storage that the website needs to work properly and securely.
- Content from other services. Some pages may load content such as images, maps or videos from third-party services, which may receive your IP address and browser information.
The Facebook, Instagram and YouTube icons on our website are links to our profiles on those services.
Your cookie choices. Most browsers let you block or delete cookies in their settings. Blocking cookies may affect how some features work.
Do Not Track. Some browsers offer a “Do Not Track” setting. There is not yet a common industry standard for how websites should respond to these signals, so our website does not currently respond to them. You can still limit cookies using your browser settings and the Google Analytics opt-out add-on described above.
5. How we share information
We do not sell, rent or trade your personal information, and we do not share it with third parties for their own marketing.
We share information collected through the website only as needed:
- Service providers that help us run the website and communicate with you, such as website hosting and software, form and spam-protection, analytics, email and text-messaging providers. They may use the information only to provide services to us.
- Legal reasons: when required by law, subpoena or court order, or when needed to protect the rights, safety or property of our patients, our practice or others.
- Business changes: if our practice is involved in a merger, sale or similar transaction, subject to applicable law.
How we use and share your medical information is described in our Notice of Privacy Practices.
6. Security and retention
Our website uses encryption (HTTPS) to protect information sent between your browser and the website, and we use reasonable administrative, technical and physical safeguards to protect information. However, no method of sending or storing information over the internet is completely secure, which is one reason we ask you not to include medical details in the contact form, texts or regular email.
We keep information collected through the website for as long as needed for the purposes described in this policy or as required by law.
If a security breach affects your personal information, we will notify you and, where required, government authorities, as required by applicable law. This includes the federal HIPAA Breach Notification Rule for medical information and state breach-notification laws such as North Carolina’s Identity Theft Protection Act.
7. Children
Our website is intended for adults and is not directed to children under 13. We do not knowingly collect personal information through the website from children under 13. If you believe a child under 13 has sent us personal information through the website, please contact us and we will delete it.
8. Links to other websites and services
Our website links to websites and services we do not control, including our patient portal (operated by a third-party health technology vendor), our Facebook, Instagram and YouTube pages, and Google Maps. Their own privacy policies and terms apply when you use them, so please review them before sharing information.
9. Your choices
- Email: unsubscribe at any time using the link in any email from us, or by contacting us.
- Text messages: reply STOP to any text message from us to stop receiving texts, or tell us that you prefer not to receive them. Message and data rates may apply.
- Access, correction and deletion: you can ask us to access, correct or delete personal information collected through the website. We will respond as required by applicable law, and we may need to keep some information when the law requires it.
- Cookies and analytics: see Cookies, analytics and similar technologies.
- Medical records: your rights regarding your medical information, such as access, amendment and an accounting of disclosures, are explained in our Notice of Privacy Practices.
10. North Carolina and Wisconsin residents
We serve North Carolina patients in person and by telemedicine, and Wisconsin patients by telemedicine only. Depending on where you live, state law may give you additional privacy rights. We will honor those rights as required by law.
11. Changes to this policy
